Using our secure portal you will be able to:

Although this tool is designed to be a convenient resource for our patients, it should not take the place of any important communications about your care. We continue to encourage you to call our office as necessary. Patient Passport does not replace Spruce secure communication.
How to Sign Up
1. Look for an email from Elation Passport Support in your email inbox. The invitation was sent to the email we have on file.
2. Click the button to “Sign into Passport” inside the email and you will be directed to the registration web page.
3. Choose a password and enter it twice in the fields labeled “Password.” Do not share your password with anyone.
4. Enter your Passport invitation code in the field labeled “Invitation Code.” The invitation code will be sent to your mobile phone via an SMS text message and is also printed at the bottom of this Spruce message.
If you have not received an email for Passport access, please contact our office by phone or via Spruce.
